Eligibility​ for Elysian Fields Program

   

You must be a Veteran, or a Veteran with a family, who is homeless and has an insatiable desire to change your current life style to a more positive, healthy, and dignified one.

You must make a rock solid commitment for yourself to work with

your Case Manager to create  achievable and

​measurable personal goals.



You must be willing to discuss needs, potential barriers, 

and  any concerns.   If you don't tell us, we won't know.
